Because Myrtle has Irritable bowel syndrome - that's why she's called "Moaning"

What was Hagrid knitting while in the subway with Harry?

He had a wicked case of the munchies and couldn't find Pizza Hut or Domino's.

You have to knock ... whisper the words "it's me" and the door will grant you access.
(mind you - the door has audio recognition; one must be a student or professor at Hogwarts)

What did Harry do with the Elder Wand?

How many times did Harry have the sorting hat on his head (during his stay at Hogwarts)?

He actually never had it on his head. As with placing him with family members, Dumbledore thought it in Harry's best interests to just be placed in the House it would be easiest for Dumbledore to keep an eye on him, so he came to Hogwarts knowing he'd be in Gryffindor. Other than Start of Year, there were no other opportunities to try The Sorting Hat on.

Why was Harry's Potions book during his sixth year in such poor shape?

It's a form of rubber duck that likes playing in the Vanishing Cabinet going from bath tub to bath tub checking out the bubbles.
